What is your company going to make? Please describe your product and what it does or will do.

We run a medical group that serves complex, low-income patients on Medicaid, Medicare, and public assistance. Our product is a vertically integrated AI care manager that supports clinical, behavioral, and social needs like housing and transportation, which are critical to health but often overlooked. The system helps care teams coordinate and manage these overlapping factors for each patient.

Where do you live now, and where would the company be based after YC?

We live in San Francisco, CA and plan to keep the company based here with a remote-friendly model.

Explain your decision regarding location.

San Francisco gives us access to AI talent and healthcare innovation. Remote work lets us tap expertise nationwide, which is critical for scaling a complex healthcare service focused on low-income populations.
