Less than 1% of patients with diabetes receive needed training. We provide a SMS-based telehealth platform to people with chronic disease on Medicaid. Our proven Healthy at Home program uses 2-way SMS interactions, weekly diabetes coaching visits, and evidence-based health education. Our vision is to reach millions of Americans that lack at-home support for chronic disease. We're billing health insurance $186 for telehealth visits and have 2 contracts underway. With your support, we can be a fully sustainable nonprofit by 2023.

What is your company going to make? Please describe your product and what it does or will do.

We built Healthy at Home, an SMS-based telehealth platform that provides weekly diabetes coaching, two-way SMS interactions, and evidence-based education for people with chronic diseases on Medicaid. It supports underserved patients who otherwise get almost no training or support.

Where do you live now, and where would the company be based after YC?

We live in San Francisco and work remotely. We plan to base the company in San Francisco after YC.

Explain your decision regarding location.

San Francisco is where we currently live and have access to healthcare networks and potential partners. Being remote lets us work across locations, but we want to stay here after YC for the ecosystem and connections.
